            I, the family representative, declare that:

            • the information in this nomination is true and complete to the best of my knowledge;
            • I have the agreement of the family members named, and of a parent or guardian for any child, to share their details, photos and videos with the MSS Usra Award team and jury;
            • the award team may contact the family and the people named to verify this nomination;
            • the jury's decision is final; and
            • if the family is selected, MSS may use the family's name, story and photos to present and promote the award, as agreed with the family.
